FMCSA medical exemptions — only two programs still exist

The $49 Medical Card Kit — $49, one-time payment

The checker reads your verdict. The Kit gets you through the exam: documentation packets for your condition, the two 45-day form clocks run as a calendar, the official medication form packet, waiting-period calendars, failure recovery, and the examiner phone script.

What the federal text says

As of October 2026, FMCSA runs exactly two medical exemption programs: hearing and seizure disorders. The vision exemption program was removed in 2022 (replaced by the alternative vision standard, §391.44) and the diabetes exemption program was removed in 2018 (replaced by the insulin rule, §391.46). FMCSA states decisions within 180 days of a complete application, and no fee appears in 49 CFR 381.310. For every other condition, there is no exemption to apply for — the standard itself, or an examiner-level form, is the whole road.

What that means for you

If a website offers to help you apply for a vision or diabetes exemption, it is selling a dead program. The live doors are narrower and specific — hearing, and seizures — and this page is the map of what actually exists.

What moves the outcome

Match your condition to the live list first. If yours is not on it, your page in this matrix names the mechanism that does apply (a standard, a form, or an SPE certificate).

Which mode is this?

This question is decided by the federal text itself. The verdict above is computed from the cited words — your examiner is bound by the same text.

Only a certified medical examiner on the National Registry can certify you. FreightVerdict reads the published federal standards — it does not certify, diagnose, or treat, and nothing here is medical advice.
